In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

  • Your medical history including a list of your current medications
  • X-rays on CD or film relevant to your problem.
  • Information about previous treatments and medications that you have tried.
  • Medical records from other medical providers that you have seen for this problem.
  • Insurance information and card.
